This survey is being conducted by Greater Sacramento Economic Council (“Provider”).

Your participation with the survey is voluntary, and you may choose not to participate. Participating in this study may not benefit you directly, but it will help us learn more about the Subject.
If you complete the survey, you will have the option to be entered to win a gift card for your time.

The information you share with us if you participate in this survey will be kept confidential in accordance with applicable law.
If you are given an opportunity to submit free-form responses to a survey question, please provide relevant and responsible responses: refrain from disparaging, offensive, harassing or otherwise inappropriate language; and refrain from including sensitive information of any kind.
If you do not wish your response and the information it contains to be processed or stored by the Provider, please do not respond to the survey.
